What this covers

This policy explains how CEREVITY, Licensed Clinical Social Worker, P.C. (“the practice,” “we”) handles information collected through privatepaytherapist.com and through questions and bookings made on it.

Health information created during your care is protected by HIPAA and California’s Confidentiality of Medical Information Act, and is governed by the practice’s Notice of Privacy Practices, which you receive before your first session. Where the two differ, that notice controls.

What we collect

When you book
Where you will be during sessions, whether the session is for you or a couple, your profession, city, name, phone number and preferred language.
When you ask a question
Your name, mobile number, your question, and your consent to a text reply.
When you schedule and pay
Details you enter in the practice’s scheduling system, including payment, which is handled by its payment processor.
When you visit
Standard server data such as IP address, browser and pages viewed, used for security and to count visits. Our analytics do not use cookies.

How we use it

  • To confirm bookings, prepare for your first session and send reminders.
  • To reply to your question by text.
  • To keep the website secure and working.
  • To meet legal, licensing and record-keeping duties.

What we never do

  • Sell or rent your information.
  • Share anything with an insurer or an employer.
  • Run advertising trackers or retargeting pixels on this site.
  • Send marketing texts, or add you to an email list.
  • Share your mobile number or texting consent with anyone for their marketing.

Who we share it with

Only the providers that run the practice, each limited to its own job and bound by a written agreement, including a Business Associate Agreement wherever health information is involved:

Jane
Scheduling, payments, records and secure video.
ClickSend
Text message replies, reminders and confirmations.
Cloudflare
Website hosting, security and cookie-free visit counts.
Google Fonts
Delivers the website’s typefaces, which shares your IP address with Google.

We also disclose information when the law requires it, such as a court order or mandatory reporting, and otherwise only with your written permission.

If you choose “Another state” when booking, you leave this site for CEREVITY’s intake. We send none of your details; the link carries only a tag showing the visit came from this site.

Text messages

With your consent, the practice texts you from (833) 519-3262 to reply to questions and to send appointment confirmations, reminders and scheduling messages. Message frequency varies. Message and data rates may apply.

Reply STOP to opt out, or HELP for help. Opting out does not cancel appointments; you can still manage them in the client portal. Text messages are not end-to-end encrypted, so please keep health details out of them.

Cookies and Do Not Track

This site sets no advertising or tracking cookies, and visit counts are taken without cookies. Because we do not track you across other sites, browser Do Not Track and Global Privacy Control signals do not change how the site behaves.

How we protect it

Traffic to this site is encrypted, providers that handle health information operate to HIPAA security standards, and access is limited to the people who need it to run the practice. No system is perfectly secure, so please do not send health details through the website or by text.

How long we keep it

Website questions are kept only as long as needed to reply and keep scheduling records. Clinical records are kept for the period California law requires.

Your choices and rights

You can ask to see, correct or delete the information you gave through this website, and we will respond within 30 days. Requests about your clinical record, including copies, amendments and an accounting of disclosures, follow your rights under HIPAA as described in the Notice of Privacy Practices.

You will not be treated differently for making a request.

Children

The practice serves adults 18 and older and does not knowingly collect information from children.

Changes to this policy

When this policy changes, the date at the top of this page changes with it. Material changes are shared with current clients directly.
